Job Title: Project INSPIRE Program Manager

Organization: Fund for Public Health in New York – NYC Department of Health and Mental Hygiene

Position Reports to: Director of Office of Viral Hepatitis Surveillance, Prevention and Control, Bureau of Communicable Diseases, NYCDOHMH

The Fund for Public Health in New York, Inc. (FPHNY) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNY implements programs to address pressing public health needs, fosters private sector support to enhance health and health care, and helps educate the public regarding the protection of individual, family and community health.


The Fund for Public Health in New York (FPHNY) in collaboration with New York City Department of Health and Mental Hygiene (NYCDOHMH) received a three-year award from the Health Care Innovation Awards starting in September 2014. This opportunity, Innovate and Network to Stop HCV and Prevent complications via Integrating care Responding to needs and Engaging patients and providers (INSPIRE Project), will significantly expand DOHMH’s activities in addressing the holistic care for Hepatitis C.  

Project INSPIRE NYC is an integrated, innovative, and evidence-based comprehensive service model that combines the use of HCV surveillance data, facilitated case identification, care coordination, health promotion, medication adherence support, and HCV infection-centered primary care that is supported by expert consultation via telemedicine and that includes collaborative behavioral health services. The primary aim is to demonstrate a model of service delivery and payment that can reduce morbidity and death from chronic illnesses and reduce costs associated with its complications, using chronic HCV infection as a case study. Project INSPIRE NYC will use an integrated medical and behavioral health service model with a bundled and capitated payment system developed with participating managed care organizations (MCOs). It will create real value by standardizing and improving patient-focused care, while working toward a payment model that facilitates positive outcomes and cost efficiency.

We are seeking a seasoned healthcare leader (6+ yrs exp.) who has programmatic start up and implementation background, preferably in the hepatitis field


  • Oversee the day-to-day operation of Project INSPIRE

o   Ensure that goals are met

o   Coordinate activities with partners

o   Supervise up to 4 staff

  • Work with clinical partners to ensure that program components and activities are implemented according to the work plan

o   Oversee management of project activities (eg., integrated care, care coordination and telemedicine)

o   Liaise between project partners to ensure that patients are flowing through the system according to protocol

o   Provide training on project protocols

o   Oversee collection of program data and reporting to partners and funder

o   Work with Program Evaluation staff and financial manager to ensure appropriate data are collected and program evaluation, monitoring and reporting are proceeding according for work plan

o   Oversee preparation of progress reports

o   Work with partners to prepare results and publish findings

o   Other duties as needed to fulfill grants objectives

  • Oversee the monitoring of contractors:

o   Ensure contractors are performing required deliverables as stated in their contracts, according to the designated timelines.

o   Provide technical assistance related to the proper completion of the deliverables, and ensure they maintain the capacity requirements as stated in the contracts.

o   Ensure invoices from contractors are reviewed and discrepancies investigated.


  • Masters Degree(s) in Public Health
  • At least 6+ years programmatic experience in the public health or health care fields, preferably in hepatitis C
  • Strong communications  (written and verbal) skills
  • Highly organized with strong attention to detail
  • Strong ability to work with partners or various types
  • Ability to travel throughout NYC ~ 50% of the work week
